Preparing For Your First Tele-Mental Health Appointment (NEW CLIENTS)

IMMEDIATE APPOINTMENTS:

  1. If you request to speak with a counselor in the next 10-15 minutes, the receptionist will schedule an appointment for you and ask you to log into the Patient Portal to complete the pre-session paperwork prior to speaking with the counselor.  Log into the Patient Portal by clicking the image below. 

      

 

    2. Click on "Check-in for an Appointment," and then verify your phone number/mobile carrier/address are correct, complete the consent forms, the pre-session questionnaire, and the CCAPS survey.                         
   
* A special note regarding the UCC Telecounseling 20 Form:  This form includes a section that requires that you list a client support person, a second client support person or professional contact, and a local hospital.  Your FIRST CLIENT SUPPORT PERSON should be someone who is located in or near the city/town where you currently live.  The SECOND CLIENT SUPPORT PERSON/PROFESSIONAL CONTACT may be a trusted person who is not local.  The preferred hospital that you list should be local.  Details for the local Corpus Christi hospitals that provide emergency mental health care are listed below.

3. Once you have completed the pre-session paperwork, you will see a prompt that says "Join Meeting" which automatically directs you to the Zoom videoconference room.  DO NOT click this link; wait for the counselor to call you.

INITIAL APPOINTMENTS SCHEDULED IN ADVANCE:  

1.  Prior to your scheduled appointment, log onto the Patient Portal to complete the required paperwork.  Click on the image below to access the Portal.

2.  Once you are in the Portal, go to the PROFILE tab in the left toolbar and verify that your address and phone number and other identifying information are correct.  To enable text message appointment reminders, you will need to verify your mobile phone number, check "I would like to receive text messages" and add our phone carrier.

3.  Click on the FORMS tab in the left toolbar and complete the following forms:

  • UCC Informed Consent 20
  • UCC Notice of Privacy Practices 20
  • UCC Telecounseling 20     *The first client support person should be someone who is located in or near the city/town where you currently reside.  The second client support person/professional contact can be a trusted person who is not local.  List a local hospital.  (Details for the two local Corpus Christi hospitals is listed above).
  • You do not need to complete the other forms that you see listed in the Portal. 
      4.  On the date of your scheduled appointment:  At least 5 minutes prior to your scheduled appointment    time, make sure that you are in a private location where you will not be interrupted. Log into the Patient Portal.  Click on "Check-in for an Appointment."  Once you have completed the pre-session paperwork, you will see a prompt that says "Join Meeting" which automatically directs you to the Zoom videoconference room.  DO NOT click this link; wait for the counselor to call you. 

 

 General Instructions for Tele-Mental Health Appointments (RETURNING CLIENTS)

  1. Log into the Patient Portal by clicking on the image below. (You may want to bookmark the Portal site for future use). You can use any device with a webcam for videoconference appointments.     

 

  1. From the Portal home screen, click on "Check-in for an Appointment," and then verify your phone number/mobile carrier/address are correct, complete pre-session questionnaire(s), and complete the CCAPS survey. (see below graphic)
  2. Then click "Join Meeting" and wait for your counselor to start the Zoom session.
